Ventilator Weaning

August 10, 2020- A systems-based project to standardize evidence based ventilator weaning protocols has been approved with the input and review of McLaren physician leaders on the Critical Care Task Force and HVC Steering Committee.

WHY: Standardizing Spontaneous Awakening Trials (SAT) and Spontaneous Breathing Trials (SBT) to ensure the process of sedation management in adult mechanically ventilated patients in the ICU is appropriate through the use of sedation scales which aim at targeting light sedation. Evidence based protocols and have been shown to decreased time on mechanical ventilation and improve patient outcomes.

WHAT: Implement evidence-based protocols and require proper documentation to be entered into the EMR.

HOW:

  • Education (flowchart – SBT/SAT Protocol – Wake-up and breathe)

  • Audit and Feedback (review by HVC teams, ICU teams)

  • Clinical decision support interventions

    • Defined daily processes, protocol, templates in EMR

    • Standardizing documentation for data extrapolation.

Choosing Wisely Recommendations

Critical Care Societies Collaborative – Critical Care

Don’t deeply sedate mechanically ventilated patients without a specific indication and without daily attempts to lighten sedation.
